Appointments


Please call (330) 602-8833 between 8:00 AM and 5:00 PM, Monday through Friday to schedule an appointment.

If you are a new patient, you will be asked to fill out New Patient Forms. Please download, print, and complete the forms.  You are encouraged to complete these forms and return them our office prior to your appointment. You can fax them to us at 330.602.8832, drop them off at the office anytime Monday through Friday 8 to 5, or scan and email them to reception@entallergyandsinus.com. Otherwise, please bring your completed forms with you to your appointment.

Cancellations: If you cannot make it to your scheduled appointment, please call the office as soon as possible to cancel (24 hours notice is appreciated).  If it is after hours, the answering service will take a message and pass it on to the receptionists in the morning.

Insurance & Billing

We accept most major health insurance carriers (For example: Aultcare, Aetna, Anthem, Cigna, United Healthcare, Medical Mutual, Ohio Health Choice, etc.  This is not an exhaustive list).  Please call the office to verify that we are contracted with your health insurance carrier.  Additionally, we recommend that you check with your insurance carrier regarding what services they will or will not cover.  If certain services are not covered, you are responsible for payment at the time of service.

We accept Medicare and traditional Medicaid, as well as Buckeye, Care Source, and Unison.

Please read our Financial Policy for more detail on all issues related to Insurance & Billing.  Additionally, our billing coordinator is available to speak with you from 8 to 12 and 1 to 5 Monday through Friday.

You will be considered a self pay patient if you are not covered by any insurance policy or third party payer, if you are covered by insurance plans in which the office does not participate, or if you do not have an insurance card on file with us.  Self pay patients are required to bring $125 at the initial appointment and will be asked to make payment arrangements for the balance.  Self pay patients will receive a 40% discount across the board for all services rendered.

CareCredit

Financing is available for hearing aid purchases through CareCredit.  Many options exist, including 0 percent financing for one year.  Dr. Justin Gherian, our audiologist, can speak with you about CareCredit options.  You can also visit their website at www.carecredit.com.
