Please call (330) 602-8833 between 8:00 AM and 5:00 PM, Monday through Friday to schedule an appointment.

If you are a new patient, you will need to complete New Patient Forms. Please download, print, and complete the forms.  The forms must be returned to our office at least 24 hours prior to the appointment date. If the forms are not received, you may be asked to reschedule the appointment. You may drop the completed forms off at our office Monday through Friday from 8-5, mail them back to us at the address above, fax them to 330-602-8832, or scan and email them to

Information About Your Appointment:
– Please arrive 15 minutes prior to your scheduled appointment time.
– Bring your insurance card(s) with you.
– If applicable, your copay will be collected.
– As a new patient, please expect your first appointment to last at least an hour. This is because Dr. Hartman will need to do a complete assessment of your condition.
– If you have undergone any medical tests that are related to why you are seeing Dr. Hartman, please arrange for these test results to be sent to our office. If you need assistance with this, please call the office.
If you need to cancel your appointment, please call our office at least 24 hours before your scheduled appointment. You can call any time as you will either reach the receptionist or our answering service.


Insurance & Billing

We accept most major health insurance carriers (For example: Aultcare, Aetna, Anthem, Cigna, United Healthcare, Medical Mutual, Ohio Health Choice, etc.  This is not an exhaustive list).  Please call the office to verify that we are contracted with your health insurance carrier.  Additionally, we recommend that you check with your insurance carrier regarding what services they will or will not cover.  If certain services are not covered, you are responsible for payment at the time of service.

We accept Medicare.  However, as of August 2013, we are no longer accepting new patients with Medicaid coverage (traditional Medicaid as well as Buckeye, Caresource, and United Health Care Community Plan).

Please read our Financial Policy for more detail on all issues related to Insurance & Billing.  Additionally, our patient accounts coordinator is available to speak with you from 8 to 12 and 1 to 5 Monday through Friday.

You will be considered a self pay patient if you are not covered by any insurance policy or third party payer, if you are covered by insurance plans in which the office does not participate, or if you do not have an insurance card on file with us.  Self pay patients are required to bring $125 at the initial appointment and will be asked to make payment arrangements for the balance.  Self pay patients will receive a 40% discount across the board for all services rendered.


Financing is available for hearing aid purchases through CareCredit.  Many options exist, including 0 percent financing for one year.  Dr. Justin Gherian, our audiologist, can speak with you about CareCredit options.  You can also visit their website at
